It is important to do the annual cycle of care to identify health concerns early and discuss the best possible treatment.
Diabetes can develop to health problems that can affect your whole body, including your kidneys, eyes, feet, nerves and heart without regular health checks
What is included in the Annual Cycle of Care?
Health checks are undertaken of your:
- Blood glucose levels
- Blood Pressure
- Foot check
- Eye check
- Kidney health
- Weight and Waist measurements
- Amount of physical activity
- Medication review
- Smoking and drinking status
- Emotional health
Who undertakes the health assessment?
The physical observations and collection of information will be undertaken by a Registered Nurse. Following this, your doctor will see you and may request additional clinical tests be ordered.
